Thank you for requesting a screening with the Guernsey Chest and Heart LBG.

By volunteering to undertake screening with us, it is important that you read and understand the following information.

The screening will consist of the following assessments: height, weight, body composition, electrocardiograph (ECG), a blood test to measure your cholesterol and blood glucose level, and blood pressure. This information, along with the lifestyle questionnaire you completed online, will be used to inform a discussion regarding your cardiovascular health. This information is stored electronically and sent to your GP. You have the right to decline any or all aspects of the assessment at any time.

We are unable to advise on specific health conditions and only provide general information regarding lifestyle changes to reduce cardiovascular health risks.

If you have any specific questions about any medical matter, you should consult your GP.

If you think you may have any cardiac or other medical condition you should seek immediate medical attention. If you are under the care of your GP you must not rely solely on the information provided by the Screening Nurse at the time of screening. We do not have access to your medical records, and we will refer you to your GP should your results suggest further investigations are required.

You should never delay seeking medical advice, disregard medical advice, or discontinue medical treatment because of information received during screening.

Should the Screening Nurse identify an extremely high blood pressure reading the nurse will seek your consent to contact your GP. Should you decline, it is your responsibility to seek medical advice.

You should not rely on information provided for medical decision making and should direct all questions regarding medical matters to your GP or other health care provider.
